Contract

I _________________ contract with Brenda Van Noy: (Brenda’s Home Daycare) for childcare services. I understand that she will provide childcare for ________________at the rate of 135.00 per week for children 2 and above with supplementary breakfast or food items, and $135.00 per week for younger children. Holiday’s non-exempt.

I understand that children shall be dropped off and picked up promptly to avoid distraction and diverted attention.

I understand that anything over 9.5 hours per day shall be charged at a rate of $10.00 per each additional hour 1 hour minimum.

I understand that payment shall be made the morning of the first day of the business week in advance.

I understand that termination shall be immediate should a violation of law occur.

I understand that the hours of provided service shall be no earlier than 7:30 A.M. and no later than 5:00 P.M. Monday thru Friday.

I understand that in order to protect all the children from illness I may not bring my child/children to the Van Noy home for childcare should he/she be sick with fever, vomiting, diarrhea, unidentified rash or anything that would affect other children in Brenda’s care.

I understand that I will be responsible to pay the tuition for childcare space. If for any reason my child is absent I will still be required to pay the tuition. This includes vacations, sick days, doctor’s appointments, etc. Tuition is not required for Brenda’s vacation days. 

I understand that I as the Parent/Guardian am responsible for the provision of diapers, wipes, formula and all special dietary supplements, food etc.
